Judicial Candidate Voluntary Self-Disclosure Statements

1) You must save the 10-page statement form to your computer. (To save it, right-click on the form and "Save as ..." or look for the download arrow in the upper right-hand corner.) Do not type in the form without saving it first to your computer. If you try to complete the form online, your information will not be saved. Download and Save the Judicial Candidate Voluntary Self-Disclosure Form.

2) Open the saved file from your computer and type in your answers (you need Adobe Acrobat to open the document properly). To prevent accidental deletions, we recommend saving the document often. Save the completed statement form to your computer.

3) Log in and submit the completed statement form to the The Florida Bar website using this link (you will need to enter your Bar number and password) by 5:30 p.m. Monday, June 13, 2016.

You will be able to direct media and the public to your form on The Florida Bar’s website at beginning July 11, 2016.
